Private fit notes for work, signed by a UK doctor.

A private doctor's letter for your employer, not an NHS fit note. Answer a short online questionnaire and a UK GMC-registered doctor reviews it the same day.

From £32.99 · Full refund if no letter is issued

Three steps, all online.

  1. Answer the questionnaire

Choose Sick Leave from Work or Adjustments to Work Duties, then answer a short online medical questionnaire.

  2. Reviewed the same day

A UK GMC-registered doctor reviews your answers and decides whether a letter can be issued.

  3. Signed and sent, if approved

The letter is sent to you by email and SMS.

When do you need a fit note?

  1. Days 1 to 7

Employees can usually self-certify without medical evidence.

  2. Day 8 onwards

Your employer can ask for medical evidence. A private doctor's letter is one option, if your policy accepts it.

  3. Your employer checks it

HR can confirm a letter came from this service: verify a letter (opens Dr Sick's website).

Questions from employees and HR

More answers in the FAQ, or contact us.

Will my employer accept a private doctor's letter?

Employers set their own evidence rules. Some accept a signed letter from a registered doctor; others ask for an NHS fit note. Check your sickness absence policy or ask HR before you order. HR can also check a letter is genuine online.

Is this the same as an NHS fit note?

No. This is a private doctor's letter, sometimes searched for as a private fit note. It is not issued by the NHS or a GP surgery, and it is not a GP appointment. Your employer decides which evidence it accepts.

What dates can the letter cover?

The doctor decides what, if anything, to certify and which dates the letter covers, based on your answers. One letter covers up to 6 weeks. For longer absences, see your GP.

What if the doctor can't issue a letter?

The doctor reviews your answers and may decline. If no letter can be issued, you get a full refund to your original payment method within 3 to 5 working days.

The questionnaire takes a few minutes.
