What the questionnaire collects
- Your contact details, so your letter can reach you by email and SMS
- Your answers to the medical questions, which may include health information the doctor needs
- Your payment details, entered in the questionnaire's payment step
Why we use it
So a doctor can review your request and, if approved, issue your signed letter and send it to you. Payment details are used to take payment, and to refund you if no letter is issued. Your answers are treated as confidential.
Who handles your information
- Jotform provides the questionnaire and payment form.
- Cloudflare hosts this website.
- This site's analytics and advertising tags never receive your letter type, symptoms, condition or other health details.
Cookies
Google Ads sets measurement cookies only after you accept; until then it sends cookie-free signals with no health details. You can change your choice at any time with Cookie settings at the foot of every page. The questionnaire may set its own cookies. Read our cookie policy.
Your rights
Under UK GDPR you can ask to see the information held about you and to have it corrected or deleted. The full privacy policy explains how these rights apply.
