Cookie policy
What this site always stores
Your cookie choice, Accept or Reject, is saved in your browser's local storage so this site remembers it next time. It is not used for tracking or advertising.
Tags and when they load
This site uses one tag, Google Ads, to measure which ads lead to orders. It runs on every page in Google's consent mode: until you accept, it sets no cookies and stores nothing on your device. You can reject and still use the whole site.
Tag Purpose Cookies Google Ads Ad measurement Only after you accept; without them it sends cookie-free signals Jotform Runs the questionnaire and payment Only on the page where you start a letter When you complete the questionnaire, one generic “order” signal is sent to Google Ads. It contains no health details and nothing you entered in the form. A flag in your browser's session storage stops the same order being counted twice; it is deleted when you close the tab. We don't build advertising audiences from visits to this site, ad personalisation is off, and our host, Cloudflare, runs no analytics script on these pages.
The questionnaire's own cookies
The questionnaire may set its own cookies and run its own Google and Meta tags. These don't depend on this site's banner, so rejecting here does not switch them off.
Blocking cookies in your browser
Most browsers let you block or delete cookies and site data. Blocking them may stop the questionnaire working, and clearing site data brings the banner back on your next visit.
Change your choice
You can reopen the banner and change your choice at any time. Choosing Reject switches Google Ads back to cookie-free signals and removes accessible Google measurement cookies and stored ad-click data. Cookies belonging to the embedded questionnaire are controlled separately.
